INSTRUCTIONS FOR APPLICATION

The admission process begins with a review of the completed application package by the Department of Physical Therapy Admissions Committee.  Therefore, all application packages must be sent to:

Phenix Hall, Room 216
Hampton, VA 23668

The application package must include:

  • Completed Graduate College application PDF link
    • Two letters of recommendations (letters from teachers and physical therapists only)
    • Personal Statement
  • Volunteer work form(s) PDF link
  • Prerequisite Course Form PDF link
  • Application fee: $25.00 money order made out to Hampton University
  • Official Transcripts from all colleges attended
  • Photo Copy of Graduate Record Examination (GRE) score or hand written score in a separate sheet with dates.

Note: Only completed application packages will be reviewed and forwarded to the Graduate College.
