The Student Health Injury and Sickness Plan is designed to meet the health care needs of the diverse student population attending Hampton University. The plan provides the student with comprehensive coverage to include prevention and wellness benefits. The annual premium for the 2012-2013 year for undergraduate students is $844 per year. The annual remium for graduate students $963 per year. The coverage period is August 2, 2012 to August 1, 2013. Undergraduate and graduate students may select a plan for spouses and dependents. Please visit http://www.gallagherkoster.com/hamptonu to enroll or waive the student health insurance.
The plan carrier is Companion Life Insurance Company, a subsidiary of Blue Cross Blue Shield of South Carolina. The account manager is Gallagher Koster. They can be reached via telephone from 8:30am to 6:30pm EST at 877-380-4347 or by email, hamptonustudent@gallagherkoster.com. All claims are filed to Klais & Company, 1847 West Market Street, Akron, OH 44313. They can be contacted at their website, www.klais.com, or by calling 1-800-349-9017. The plan provides health insurance coverage anywhere in the United States and when students are traveling abroad. The plan is an indemnity plan and can be used at any physician office, hospital, or urgent care center.
The plan might pay, in addition to other insurance coverage, when an out of state HMO does not provide coverage and might assist with the deductibles or any unpaid balances that are not covered by the other insurance. All students with a limited, out of area coverage, or otherwise inadequate coverage in the Hampton Roads, VA area, will need to enroll in the student insurance plan.
