How can I Request a Certification of Enrollment?
If you need certification of enrollment for your lender, insurance
company, etc., you must send a written request to the Student Records
Office. If you have a form to be completed, please fax the form (or
a copy) to 757/727-5095 or mail to :

Hampton University
Hampton, VA 23668
To aid in processing your request, please include the following
information:
- Name as it appears on your Hampton record
- Student Number ( usually social security number )
- Date of Birth
- Dates of Attendance
- Address/fax number to which you want the certification sent
- The student must sign the request ( computer generated signatures
are not acceptable.)
- If this information is being provided for dependent insurance
coverage, the name, social security number or policy number of
the employee should also be provided.
