How can I Request a Certification of Enrollment?
If you need certification of enrollment for your lender, insurance
company, etc., print out and complete the VERIFICATION REQUEST FORM. Then bring the completed form to the Office
of the Registrar, fax the form to 757-727-5095 or mail to the
address below.
If you have a form to be completed by the Registrar, please fax the form to 757-727-5095 or mail a copy to :

To aid in processing your request, please include the following
information:
- Name as it appears on your Hampton record
- Student Number ( usually social security number )
- Date of Birth
- Dates of Attendance
- Address/fax number to which you want the certification sent
- The student must sign the request ( computer generated signatures
are not acceptable.)
- If this information is being provided for dependent insurance
coverage, the name, social security number or policy number of
the employee should also be provided.
