request AV support for events on campus (OUTSIDE OF THE UNIVERSITY
AND CAMPUS CENTERS), please fill out this online Form. A contact
will get in touch with you to further discuss costs, or any specific
instructions for the event. Please allow at least 48 hours for AV
request and 5 days in advance for videotaping and special sound
requirements per event. If you DO NOT hear from anyone within 2
days please contact, Walter Walker, at the above phone number.

Responsibilities |
- The
BORROWER accepts responsibility for the equipment listed
while is his/her possession of that anyone designated by
him/her ON BEHALF OF THE BORROWER'S DEPARTMENT, PROGRAM,
OR CENTER. IN THE CASE OF any loss or damage incurred during
the use, transport, or storage of this equipment once it
has been taken from the Center (normal wear and tear excepted),
THE DEPARTMENT, PROGRAM, OR CENTER WILL BE HELD RESPONSIBLE
FOR THE REPLACEMENT OR REPAIR OF BAD EQUIPMENT. PROMPT return
of this equipment IN GOOD CONDITION is expected.
- When
equipment is being shared on a day-long basis by a group
of instructors, the instructors from the first and last
sessions of the day will be held responsible for the equipment
and will both be included on the request form.
- If
you are a student, your professor or Instructors must request
the using of this equipment in fulfillment of a class project
or assignment.
